Paramedic/ANP - Armada Family Practice

Job summary:

Armada Family Practice have an exciting opportunity for an enthusiastic and motivated Paramedic or ANP to join our friendly and supportive practice team for 16 hours per week.

We are seeking an experienced Paramedic or Advanced Nurse Practitioner with an interest in frailty to join our multidisciplinary clinical team who can work effectively and autonomously to undertake patient assessment, diagnosis and clinical management of patients within a care home setting.

 

Main duties of the job:

You will be working to support our Partners and Salaried GPs to deliver a high standard of patient care as an Advanced Practitioner in general practice. Using advanced autonomous clinical skills and a broad in-depth theoretical knowledge within the scope of practice of the post, the post holder will be able to assess and treat both acute and chronic diseases in a care home setting.  The post holder will undertake a weekly ward round at each of our care homes, assessing new residents and acute/chronic problems discussing patients with the practice team to ensure ongoing care needs are addressed.  Liaising with patients and their family as needed and ensuring completion of care plans.

The practice has a diverse team of GPs, Advanced Practitioners, Paramedics, Practice Nurses, HCAs, Pharmacists, MSK specialist, Dementia Nurses & Health and Wellbeing coach and is supported by a PCN mental health nurse and social prescriber. 

We are a training and teaching practice and we host/mentor GP registrars, foundation doctors and medical students.

All our appointments are booked by our experienced care navigation team who ensure that patients are booked in with the right clinicians to help the patient. They use a navigation tool called Sentiers which has been adapted to meet our needs, and our appointment system.
