Clinical Pharmacist - Hanham Health

An exciting and unique opportunity is available for a Clinical Pharmacist, to deliver a wide range of pharmacy services, as part of the Hanham Health Clinical Team across 2 of our GP Practices in South Gloucestershire. The role will involve face to face consultations with patients as well as medication reviews and queries via the telephone.

General Duties

• To reconcile medicines following hospital discharge and work with the Hanham Health Prescribing Team, patients, and community pharmacists to ensure patients receive the medicines they need post discharge.
• To provide pharmaceutical medication review and health education advice to patients at the designated practices by face to face, telephone or by notes-based review as agreed with the practice.
• To provide pharmaceutical input to long term conditions clinics as appropriate
• To hold medication review clinics and facilitate implementation of changes appropriate follow-up monitoring.
• To develop and run pharmacist-led clinics to monitor the effects of medication by checking clinical signs and symptoms where medicines are the main intervention e.g minor ailments, anticoagulation, asthma, diabetes, or blood pressure.
• To contribute to public health campaigns, including flu vaccinations, childhood immunisation and adult immunisation programmes
• To act as a source of medicines information for all the practice team and patients, e.g around doses, side effects, adverse events, possible alternatives (around out of stocks).
• To improve prescribing practice though educational support for all prescribers within the designated practice.
• To lead on areas where changes in evidence require changes in prescribing across patient populations, e.g where a drug is withdrawn, or indications change.
