Integrated Primary Care and Community Pharmacist - The Old School Surgery

Job Summary

We are seeking a dedicated Integrated Primary Care and Community Pharmacist to join our healthcare team.

An exciting and unique opportunity has arisen for a full-time pharmacist to develop and evolve a 'shared workforce' model to help meet the needs of our local population.

This innovative 'shared workforce' position will be a joint cross sector role across both our primary care team and our on-site community pharmacy, realising the untapped benefits of integrated care.

The successful candidate will have access to a varied portfolio career, strengthening cross sector working, and enhancing the benefits of integrated care to facilitate our vision of whole system patient care.

Fully funded education and training of primary care pharmacy education pathway course (PCPEP) will provide the candidate with transferrable consultation and prescribing skills to enable the candidate to work across all settings.

The Old School Pharmacy is co-located within the Old School Surgery and is a well-established independent community pharmacy with a strong professional relationship with our surgery.

The successful candidate will have the opportunity to be a second pharmacist (0.5 WTE) within the Old School Pharmacy.
