Practice Nurse - Fallodon Way Medical Centre

An exciting opportunity has arisen for a Practice Nurse at Fallodon Way Medical Centre to join our experienced and dedicated nursing team. The team consists of 3 practice nurses, 2 assistant practitioners and 3 HCAs.  It is expected that the successful applicant will be responsible for one area of long-term condition care. We are particularly interested in hearing from candidates competent in diabetes care. 

We are a friendly, successful, training and research practice of over 11,500 patients, located in Henleaze, which is a leafy inner suburb of Bristol, with good schools and amenities. We have a relatively high elderly population but also a good mix with families. We encourage and support ongoing professional development.

The practice combines the best of traditional and innovative working, currently with 5 partners and 4 associate GPs.  The practice is both proactive and reactive about managing the ever-changing workload in General Practice, offering a supportive environment and good work-life balance.

The practice is supported by ANPs, a Pharmacy team including two Practice Pharmacists, an Award-winning Care Coordination team, a Social Prescriber, First Contact Physio and mental health practitioner. There is a team of workflow administrators assisting the clinical team in managing inbound correspondence and results.

Job summary:

To provide an efficient, but high, standard of professional nursing care for patients. By working alongside other clinicians, to ensure patients are accessing the most appropriate level of care.  The duties will include all tasks normally undertaken by an experienced RGN and in addition any roles agreed between the nurse and the doctors as appropriate, having regard to current training.

 

 

Job Responsibilities:

· Management of chronic diseases, particularly diabetes, to encourage patients to achieve a good quality of life and to meet QoF requirements.

· Provide family planning.

· Provide routine injections and vaccinations, from baby clinics through life to pneumovax for the elderly.

· Treatment room procedures ranging from suture removal to urgent ECGs.

· Requesting path tests, processing samples and reviewing results.

 

Other tasks will include:

· Maintaining  infection control standards and general tidiness / cleanliness of  treatment rooms.

· Managing stocks of meds, vaccines and dressings.

· Maintaining contemporaneous, accurate records.

· Maintain own training to meet professional and practice standards and personal development.

· Participation in administrative systems in the practice.

· Undertake home visits as required.
